STEP 1: COURSE REGISTRATION

        Policies-Waiver:

1. The massage student shall: Demonstrate commitment to provide the highest quality massage to those who seek their professional service. Acknowledge the inherent worth and individuality of each person by not discriminating or behave in any prejudicial manner with clients and/or colleagues. Accept responsibility to do no harm to the physical, mental and emotional well-being of self and clients. Acknowledge the confidential nature of the professional relationship with clients and respect each client’s right to privacy. Conduct all business and professional activities within their scope of practice and project a professional image.
   
   
2. Cancellation of class: We understand that circumstances may arise that require you to cancel. Registrations cancelled 30 or more days prior to the workshop start date are refundable less an administrative fee of $100.00 or you may transfer to another workshop within twelve months. Registrations cancelled within 30 days of the workshop start date are non-refundable, however can be transferred to another workshop within twelve months.
   
   
3. Audio and/or videotaping is not permitted.
   
   
4. RELEASE OF LIABILITY AND INDEMNIFICATION: CDN Eldercare Massage Corp. assumes no liability for any cancellation, injury, loss, accident or delay occasioned by the proprietor, employee, or service of any facility used by the above program.
   
   
5. Without limiting the foregoing and in consideration of my being permitted to participate in the above program, the undersigned does hereby for himself/herself, his/her heirs, executors, administrators, successors and assigns, release and forever discharge CDN Eldercare Massage Corp. and their respective directors, officers, agents, employees, administrators, heirs, executors, successors and assigns from any and all actions, claims or demands by reason of any loss or injury to person or property arising from the operation of the above program, notwithstanding that the same may have been contributed to or occasioned by the negligence of CDN Eldercare Massage Corp. and their respective employees and agents.
   
   
6. I further agree to save harmless and indemnify CDN Eldercare Massage Corp. from and against all claims, actions, costs or expenses in respect to injury, loss or damage to person or property howsoever caused arising out of or in connection with the operation of the above program, whether the same may have been contributed to or occasioned by the negligence of CDN Eldercare Massage Corp. and their respective employees and agents.
   
   
7. I hereby confirm that I have no criminal charges and/or convictions whether I have no convictions or out-standing charge for any relevant or specified offence(s) under the Criminal legislation • I hereby agree to having no vulnerable sector charges on or been convicted of and been granted any pardon for any sexual offence(s) under the Criminal Records Act. • If I am ,here often, charged with or convicted of a relevant or specified offence(s) at any time after CDN Eldercare Massage Corp. course, I further agree to report the charge or conviction to CDN Eldercare Massage Corp. and provide, in a timely manner. I hereby declare that the above statement is true to the best of my knowledge and belief, and that I understand it is made for use as evidence in court and is subject to penalty for perjury.
